I have gone through the history provided and, it could be mild grade 1 acne, resulting in the formation of white and blackheads. Due to the absence of an image and considering the history that has been provided, the small black bulging area on the face may be comedones or blackheads, which usually appear as a part of mild acne. However, in order to provide the confirmatory diagnosis for the same, I would like to request you provide an image.

Acne grade one may be due to multiple reasons, including:

  1. Genetic susceptibility, wherein there is a transmission of similar conditions from parents and ancestors.
  2. Due to hormonal imbalances wherein, there is a change in the androgen hormone levels as well as female hormone levels.
  3. PCOS (polycystic ovarian syndrome) is quite common in young females and may have to be ruled out as a cause of acne. PCOS accompanies other symptoms, including menstrual irregularities, hair fall, and unwanted facial hair growth. Even in the absence of these symptoms, due to the presence of acne, it is always advisable to consult a gynecologist in order to rule out this condition and for faster treatment.
  4. The other causes of acne and the formation of blackheads and whiteheads include climate changes, sweating, stress, using skin care products, any medications taken for any underlying medical condition, dietary factors like intake of more oily and fried food, red meat, sweets, milk, and dairy products.

Uneven skin tone may also be caused due to the reasons mentioned above and due to excess sun exposure, which releases melanin pigment, a pigment that is produced by the skin as a compensatory mechanism to sun exposure. They are also commonly seen in the case of hormonal fluctuations.

My advice:

Consult your specialist doctor, talk with them, and take the medications with their consent.

1. A face wash containing Glycolic acid, Vitamin E, and Aloe vera may be used to cleanse your face externally. Apply a sufficient amount, massage in gentle circular motions for one minute, and wash off. This may be continued to be used until the blackheads and whiteheads fade. They also help with uneven skin tone.

2. Apply Clindamycin and Nicotinamide gel externally on the blackheads and whiteheads twice daily in the morning and at night after cleansing the face with the above cleanser. This may be applied for months together for good results.

3. Take a chewable tablet of Ascorbic acid (Vitamin C) 500 mg once daily after breakfast. It may be consumed for a period of two months. They help in providing an antioxidant benefit to the skin and thereby correcting the skin tone as well as the production of acne. You should avoid extremely hot food, hot drinks as well as milk during the consumption of this medication.

4. Take Vitamin E 400 mg capsule after dinner for one month. They help in hydrating the skin as well as help with uneven skin tone and skin rejuvenation.

5. For better skin rejuvenation and reduce the chances of active acne, it is also advised to undergo procedures that help regularize sebum production and reduce bacterial acne infections. Glycolic acid chemical peels, along with Kojic acid chemical peels, may be used for applying on the face for five to ten minutes and then washed off. They are usually done in six sessions every two weeks. This procedure is done at a dermatologist's clinic, and therefore, it is advisable to consider a dermatologist in person for undergoing the procedure fixture. They help in treating uneven skin tone by regularizing melanin production and reducing the recurrence of acne.

6. It is also important to avoid diet leading to the formation of acne, including oily and fried food, milk and dairy products, red meat, and sweets. Make sure to consume more fruits, vegetables, fiber, and at least ten glasses of water per day in a 250 ml glass size.

As I have mentioned above, due to the triggering of external and internal factors which might result in the formation of acne, it is important to avoid the external factors and treat the internal factors. Acne may be on and off, and uneven skin tone may also take time to reduce; therefore, consistency with the medication is the key.

Once the current sebum production regularizes, we may manage the hyperpigmentation or the uneven skin tone in a more elaborate manner. It is not usually advisable to use any other skincare product in order to treat uneven skin tone as it may trigger the formation of acne.
