Is my sinus congestion related to HIV infection?

Hello doctor,

I had unprotected sex with a transgender sex worker two weeks ago. The sex worker had both of the testicles removed, so there was no ejaculation, and claimed to be clean. But since then, I have become very paranoid with severe anxiety.

Since I was scared, I took a PCR (Polymerase Chain Reaction) RNA (Ribonucleic acid) test ten days after the exposure, and the test result was negative. But 15 days after this incident, I suffered from sinus congestion, lightheadedness, indigestion, and loss of appetite, which I feel might be because of my anxiety. Now I am concerned about whether I am being paranoid.

Also, can I rely on the negative RNA test?

The chances of transmission of HIV (human immunodeficiency virus) by a single unprotected intercourse with an HIV-positive person are low. And if the person is HIV-negative, then there are no chances. The HIV RNA (ribonucleic acid) PCR (polymerase chain reaction) test is susceptible after ten days of exposure, but is not considered conclusive.

The chances of an HIV test coming positive later are almost nil. But it is better to go for an HIV antibody test after three months of exposure to be relieved of anxiety and for conclusive results. The symptoms are non-specific, and they need not be due to HIV. It can be due to anxiety or stress. If it persists, I suggest you consult your doctor for further examination and management. Also, I suggest you get involved in activities of your interest. Do yoga, meditation, and exercise.

Thank you for the reply.

I took another RNA test and a 4th generation test on day 17, and both were negative as well. It is day 21, and I physically feel fine besides a little dry mouth that comes and goes.

Am I just being paranoid at this point?

It is good that the HIV tests are negative again, as expected. Maybe you are too paranoid about this. Just forget the issue for now. Get involved in activities of your interest. There is no need to repeat tests again until post three months of exposure. Just go for an HIV antibodies test after three months of exposure to be relieved of anxiety totally and conclusive results.
