Hello doctor,

I have noticed a white, pimple-like spot on the corona of my penis. It is neither painful nor itchy. Additionally, there are white spots on the neck of the glans. What could be the problem? I have been applying a topical antifungal cream, Miconazole 2%, but it does not seem to be effective.

It does not appear to be a fungal infection. You might try using Clobetasone cream twice daily for a week. This could be due to contact dermatitis. Consult your specialist doctor, discuss with him or her, and take the medicines with take the medicines with their consent.

It is challenging to pinpoint the exact diagnosis. The differential diagnoses could include contact dermatitis, bacterial balanitis, among others. Please avoid using soap on that area; wash only with water and keep it dry. Apply a combination of a steroid and antibiotic cream twice daily to the lesions for 10 days. Consult your specialist doctor, discuss with him or her, and take the medicines with take the medicines with their consent.

If the lesions do not heal despite these measures, a biopsy of one of the lesions may be necessary to obtain an accurate diagnosis.

I have attached two photos. What could the white patches at the neck of the glans be? I cannot find Clobetasone as you suggested. Is there an alternative cream I can use?

I have reviewed the photos (attachments removed to protect the patient’s identity) and still suspect that this may be some form of contact dermatitis. If Clobetasone cream is unavailable, you can use Betamethasone cream as an alternative; it is equally effective. Alternatively, you can request any mild steroid combined with an antibiotic cream.

I have been using the cream for the past 10 days. After two to three days, the redness and white patches disappeared, and the pimple-like spot went away after a week. However, I am now experiencing redness on the skin and some on the penis head.

I would now suggest stopping the application of the cream to observe how the condition progresses. If you experience a recurrence, it may be advisable to consider circumcision.

This morning, when I woke up, I felt some pain while retracting the foreskin and noticed two slight cuts on the skin. It seems swollen and tight, and the area is very dry. What could be the issue, and could it be due to the overuse of the steroid cream?

The cuts on the inner preputial skin could be due to two possible reasons: preputial edema or fungal posthitis. At this point, I would advise you to stop using the steroid cream and start taking Fluconazole 150 mg for three days. Additionally, take an anti-inflammatory drug like Lornoxicam for two days. Please also check your blood sugar levels. Consult your specialist doctor, discuss with him or her, and take the medicines with take the medicines with their consent.
