Hello doctor,

My penis is having a white pimple like spot on the corona. But, it is not painful or itchy. There are also white spots on the neck of the glans. What can be the problem? I have been applying topical antifungal cream Miconazole 2%. But, it does not seem to be working. Please help.

It does not look like fungal infection. You can try Clobetasone cream twice daily for a week. This could be due to contact dermatitis.

It is difficult to pinpoint the exact diagnosis. The differential diagnosis could be contact dermatitis, bacterial balanitis, etc. Please avoid using soap on that area. Wash only with water and keep it dry. Apply combination of steroid with antibiotic cream twice daily on the lesions for 10 days. Despite these measures, if the lesions does not heal, then a biopsy of one of the lesion can be done to get the exact diagnosis.

I have attached two photos. What are the white patches at the neck of glans? I cannot find the Clobetasone that you have suggested. Any other cream can I use?

I saw the photos (attachment removed to protect patient's identity) and still I feel this is some form of contact dermatitis. If Clobetasone cream is not available, then you can use Betamethasone cream. It is equally effective. For that matter, you can ask for any mild steroid with antibiotic cream. Hope you get well soon.

I have applied the attached cream for the past 10 days. After two to three days all the redness and white stuffs gone away. The pimple like thing gone away after a week. But now, I am getting redness on the skin and also some on the penis head. Why does this happen?

On seeing the attached photos, it seems to me that you have responded well to Mometasone cream. Now, I would suggest you to stop applying the cream and see what happens. If you get recurrence, the it is advisable to undergo circumcision.

This morning when I waked up, I felt a bit of pain in retracting the foreskin, and I also noticed two slight cuts on the skin. It seems swollen and tight and the sky is very dry. What could be wrong and could it be due to overuse of the cream which contain steroids?

The cuts in the inner preputial skin could be due to two reasons. One is because of preputial edema occurred due to some reason and the other one is fungal posthitis. At this juncture, I would advice you to stop using steroid cream and start taking tablet Fluconazole 150 mg for three days. Also take an anti inflammatory drug like Lornoxicam for two days. Please check your blood sugar levels.

Same symptoms don't mean you have the same problem. Consult a doctor now!
