Hello doctor,

A few days back, I had to push too hard to pass stools. I started experiencing pain and felt better after using over-the-counter hemorrhoid ointment. Last night I was sitting for too long at a banquet. When I woke up this morning, it was extremely painful and hard to walk. Should I continue using the hemorrhoid ointment? Will the pain go away in a few days?

Kindly help.

I understand your concern. Is the pain at the anal verge (the part that connects your bowels to the outside skin at the anus)? If you have continuous pain and burning sensation at the anal verge without any swelling or lump making you scared of passing stools, it could be an anal fissure (small tears of skin around the back passage).

I suggest you to follow a few instructions mentioned below.

  1. Use Lidocaine gel (anesthetic) for local application before passing stools and whenever you feel pain.
  2. Use Glyceryl trinitrate (GTN) rectal ointment (muscle relaxant) twice daily after passing stools. Clean and dry after passing stools before the ointment application.
  3. Use laxatives to prevent constipation and avoid strain while passing stools.
  4. Sit in warm water with antiseptics for ten minutes daily to help in the healing of the anal fissures.
