Hello doctor,

For the last few days, I have been having gas issues in my stomach, and the gas is not passing. Also, while trying to pass stool, I noticed a small tear around the anus area. It is painful, and I also had red-colored discharge once. Today, I had loose motions and white-colored discharge with a little stomach pain. What should I do to manage this?

After going through your case, it seems like you are suffering from an anal fissure. Anal fissure is a type of ulcer in the distal anal canal. This condition is usually associated with hypertonia of the internal anal sphincter, which increases the traumatic effect of passing hard stools.

Your symptoms, pain when passing stool, bright red bleeding, mucus discharge, and constipation, all line up with this condition. Conservative treatment usually does the trick, though surgery may become necessary if those measures don't bring enough relief.

I would suggest the following:

  • A high-fiber diet to ease bowel movements.

  • Lactulose solution, two teaspoons, taken three times daily.

  • Drink plenty of water throughout the day.

  • Sit in warm water for around 15 minutes twice daily.

  • Apply Lignocaine gel mixed with 0.2% GTN (Glyceryl trinitrate ointment) two to three times daily.

  • Take one tablet of Naproxen sodium twice daily.

Take this regimen for at least two weeks, then come for a follow-up to see if the condition improves. In most cases, these conservative measures will resolve the issue.

If symptoms persist, surgery may be necessary, with a procedure called lateral anal sphincterotomy being the typical choice.
