Hello doctor,

Around 9 to 12 months ago, I started to suffer from anal itching. It is rather intense at night, after using stool, and during or after having intercourse or anal masturbation. I also have few lumps on my anus ring that seem to grow outside of my anus hole as an extended piece of flesh. They are very sensitive but they are odd and it really troubles me. I am normally healthy. I fear that my problem started when I was doing anal masturbation without telling my spouse. I was using anal toys and hands and am afraid that during the years I have created myself damage that I do not know how to fix and whether I should genuinely fix it. Can you please inspect the pictures I am attaching. I would appreciate if you can also help me understand how I can treat it and what I should do. Please, I am looking for a very discrete way to handle my situation since I am too concerned about talking with my spouse. Kindly help.

I read your query and can understand your concern.

I have seen the pictures attached (attachments removed to protect the patient's identity). You seem to be having candidal intertrigo. Intertrigo is an itchy, macerated red rash that occurs due to yeast. Due to continuous itching and scratching, the skin has thickened and developed into folds. Also, raw erosions have formed. Hence you have the burning sensation and pain.

I suggest the following medications.

  1. Take Fluconazole 150 mg stat, then 100 mg daily for 10 to 15 days from the next day.
  2. Take Levocetirizine 10 mg twice a day.
  3. Apply Clotrimazole, Hydrocortisone, or Mupirocin cream. All these are over-the-counter medicines. You may buy and mix these in equal quantities and apply them twice daily. Follow this for ten days, then switch to plain Clotrimazole twice daily.

Please consult a specialist doctor, discuss with them and start taking the medications with their consent.

I also suggest the following.

  1. Clean the area with lukewarm water.
  2. Do not use soap as it would irritate the area more.
  3. Every time you wash the area, please dry it completely by using a paper towel.
  4. Change your underclothes twice.
  5. Wear loose cotton underwear.
  6. Use any mild vaginal wash to clean the area if you need soap.

Anal area infection needs prolonged treatment. And please do not indulge in any sexual practice that might harm you in the long run. Right now, as the area is infected, anal stimulation is contraindicated. Also, it is always better to meet a doctor in person.
