Hello doctor,

I have severe pain in my left lower back and calf. Pain medication just mildly subsides. I have had pain for over a week now. No swelling just visible veins in the areas of pain.

Thank you for your query.

Every pain in the spine is surely not due to sciatica. It can be due to vitamin D deficiency causing para-spinal muscle weakness or early degeneration of the lumbar spine at your age. I suggest a few instructions for back pain:

  1. Avoid lifting heavy weights.
  2. Avoid arching your back, instead bend your knees to pick up something from the floor.
  3. Avoid sitting for long hours.
  4. Avoid 2-wheeler rides.
  5. Improve your posture while sitting.
  6. Try to get up from your chair every 20 minutes and take a quick walk around and then continue working. Always sit on a chair with back support.
  7. Do not get up suddenly from a sleeping position. Rather, turn to the side and then get up.
  8. Take bed rest if severe pain.
  9. Do hot fomentations- locally over the affected area – at least twice daily.
  10. Apply Oxalgin gel for local application, twice daily.
  11. Take tablet Pregabalin 75 mg once for 15 days before food every day.
  12. Take tablet Etoricoxib 60 mg twice for 10 days after food.
  13. Take tablet Rabeprazole 20 mg twice for 10 days before food.
  14. Take a tablet of Shelcal (calcium) 500 mg twice for 30 days before food.

Take these tablets only if you are not allergic to them. Do not take these tablets if you have bronchial asthma.

  1. Do physiotherapy, McKenzie back, extension exercises, back core strengthening exercises, glute strengthening exercises, and passive straight leg raising exercises.
  2. After the pain reduces, the following activities are good for you:
  3. Swimming, yoga, and walking are good.
  4. Jogging, running, and weight-lifting may aggravate back problems.
