Hi doctor,

I am diabetic. I have chronic balanitis for the past two years. Now, I have swelling under my foreskin and I am able to open the foreskin to clean the white dirt. I usually dip my penis in cold water after getting excruciating pain. But my swelling hurts me a lot as it causes unbearable pain while closing the foreskin.

Welcome to icliniq.com. I have gone through the attached photo (attachment removed to protect patient identity). You seem to have an ulcerated lesion on the glans penis along with balanitis. It can be due to fungal infection, herpes, chronic abrasion, or any other sexual disease. Please mention whether you have taken treatment for balanitis. Please mention whether you had any flare-up episodes of balanitis. Please mention whether the ulcer caused pain and it bleeds while touching. Please mention whether you had any risk of sexual exposure in the past. Please mention the duration of having this ulcerated lesion. Please mention whether you have taken any culture test from this lesion. I suggest you do a culture and sensitivity test to rule out the cause. After taking the test, you can take a course of antibiotics. If it was taken before the test, it can alter the lesion. You can consult a surgeon to have further treatment. The doctor can suggest to have circumcision as you are diabetic. For now, you can take some symptomatic treatment. You can take tablet Paracetamol 500 mg twice daily and Cetirizine 10 mg once in the night and apply Fusidic cream twice daily after washing it. You can follow some measures. Wash the area with warm saline water twice daily. Keep the area clean and dry. Wear loose cotton clothes. Do not scratch the lesion as it can lead to bacterial infection.

I have not taken any treatment. Due to tiredness and sleep, I left my penis unclean with my sperms. After doing this mistake several times, I started to have this problem. Balanitis has got increased gradually in the last 18 months. It got increased even after cleaning it with hot or cold saline water. I have tried using Cetirizine and Fusidic cream to heal my wounds. My pain is constant and it is not getting stopped. I have tried using Ketoconazole for the last six weeks. I got some relief. I have taken Fluconazole tablets once a week and Gandaki Rasayana tablets. I have never consulted a doctor. Can I do a culture test at any time?

Welcome back to icliniq.com. You should not do a culture test at any time. You can use Fusidic cream in the morning and afternoon for two weeks and apply Miconazole cream at night for two weeks. You can take tablet Paracetamol 500 mg twice daily for three days, tablet Cefadroxil 500 mg twice daily for seven days, and Cetirizine 10 mg at night. If you do not have any improvement in two weeks, then you can do a culture and sensitivity test. You can consult a surgeon to have circumcision. As you have diabetes, try to maintain your blood sugar levels to have proper healing. Consult a local surgeon or dermatologist to have a proper physical examination of the lesion and try to look for any swollen lymph nodes. Follow the measures mentioned above.

I am explaining my progress in seven days. After seven days, I can able to open my foreskin till the bottom. Before taking treatment, I had pain while opening my foreskin. I have tried using all the creams and I have applied it to the tip and not till the bottom. Now, I am able to apply till the bottom. I have another issue. I have pain while passing urine. I have pain in the lower area after passing urine. I have tried to remove everything but still, I have it for a long time. It seems to be acidic. Does my recovery get delayed due to diabetes? Kindly go through the attached photos taken before and after the healing of the lesion. I am not able to see the difference but the pain has got decreased a lot and I have tried taking Paracetamol. Do I need to change the medicine or can I continue taking it? I hope that I will be able to open my foreskin till the bottom to apply my medicines.

Welcome back to icliniq.com. I have gone through the attached photos. I am not able to see the expected improvement. So, you need to do a culture and sensitivity test soon. You have to do it for taking further treatment. Diabetes can delay the healing process. You should have some improvement after taking antibiotics. It has reduced the inflammation but it has failed to act upon the ulcerated lesions. Burning sensation during urination occurs due to two reasons. You can have it due to a present ulcer and a urinary tract infection. Kindly do a complete urine routine, culture and sensitivity from the ulcer, urine culture and sensitivity, and blood sugar levels. You need to consult a surgeon to have a circumcision and physical examination of the ulcer. Do these tests as soon as possible.
