Hi doctor,
I had unprotected oral sex three weeks back in a different country. I have been worried about STIs (Sexually Transmitted Infection) ever since. So I tested for HSV (Herpes Simplex Virus) 1 and 2 IgG (Immunoglobulin), IgM, syphilis, gonorrhea, chlamydia PCR(Polymerase Chain Reaction), and urine culture. I have been stressed about it. On applying direct powder post-exposure for seven to eight days, I started noticing balanitis, a kind of red bumps. I felt a sensation on my penis with no pain while urinating or discharging. My results are awaited, but my anxiety is killing me on this event's HSV acquisition. I also get a tingling sensation in my anus and sometimes itchy but no bumps; however, whenever I drink alcohol, the symptoms of feeling sensation seem to go away. Is this anxiety and stress or real symptoms? Please help.

I understand your concern. The chances of HIV (human immunodeficiency virus) transmission by single unprotected oral sex are almost nil unless there are bleeding wounds in the oral cavity. But STDs (sexually transmitted diseases) like herpes simplex virus can be transmitted by skin-to-skin contact or kissing or as STDs. The test report details are not mentioned. The symptoms may be due to balanitis, herpes, stress, anxiety, irritation, or other causes. You can send clear photographs of the lesion and details like duration, symptoms, distribution, other associated diseases, and treatments for clarification.

Hi,
My report is still under processing, but I am living in super anxiety till I get reports. I feel as if I have almost acquired HSV 1. I have attached a photographs of flat red bumps. I have no other symptoms outside of this. Thank you.

Though the photographs is unclear, the lesions may not be due to herpes simplex. You can send clear pictures and reports later. Instead, it may be due to irritation or friction. It would be best if you avoid using harsh soaps for cleaning. Clean the area with warm water twice daily.

I got two reports today, VDRL (Venereal Disease Research Laboratory), which was non-reactive, but urine culture has detected something.

Thank you for the query. The urine culture shows the bacteria E. coli. It need not be due to STDs. It can occur due to poor hygiene too. Drink more fluids and water. Maintain genital hygiene. You will need a course of antibiotics based on the antibiotic sensitivity report. I hope it is done along with the urine culture. You can consult your treating doctor for an antibiotics course. These cannot be prescribed online. They have to see a sensitivity report and prescribe antibiotics.

Hello doctor,
I am on Amikacin prescribed injection and feel better after that, and it seems the issue was a long-standing one. I got the rest of the results today. HSV1 and 2 IGG/IGM are negative. Chlamydia trachomatis IGG is negative. Gonorrhoea PCR (Polymerase Chain Reaction) urine is negative. I will be doing an HIV duo post 28 days next week. Do you have any other recommended or repeat tests for me?

You can go for an HIV antibodies test after 3 months of exposure for conclusive results.
