Hi doctor,

I think I have balanitis. The symptoms seem similar. I visited a physician a year ago and was advised to use Panderm Plus. No is swelling observed in the groin area. There are no small fluid-filled lesions as of now, but initially, there were a few. There are tiny ulcers on my penis tip. Currently, it is the red rash on the penis head and slit or cut-like wound on the foreskin. There is a milky discharge from the penis which has no smell, difficult clean while bathing. As I try to clean the discharge, the fire skin ruptures. I had pain initially but now very rarely. I have done blood tests several times in the past year, which were all normal. Please help.

I understand your concern.

From your descriptions, it seems you have balanitis. Balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene and overwashing, use of over-the-counter medications, and non-retraction of the foreskin. May be associated with diabetes, and candidal fungal infections. I suggest the following investigations:

1. Blood sugar.

2. Potassium hydroxide mount for candidiasis.

You can gently retract the foreskin daily and soak in warm water to clean the penis and foreskin. You can use a combination of Fucidic acid and Clotrimazole twice daily for one month. If balanitis does not respond to topical therapy, I suggest you take ketoconazole 200 mg twice daily for two weeks. Consult your doctor and take the medicines accordingly. Please revert for review with a picture for better evaluation.
