Hello doctor,

I am an 18-year-old male and I often masturbate. Now I have some sort of infection on the side of my penis and lots of itchiness around it, I am scared and I have never had sex. There is one more thing there is no infection or red spots under my foreskin. I have never suffered from such a condition and not taking any medication for the current condition. I have the following questions:

  1. What is causing it?

  2. How should I prevent it?

  3. What should I not do?

  4. What should I do?

By your explanation, it seems you have balanitis, which needs your few pictures for confirmation. Balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene and over-washing, use of over-the-counter medications, and non-retraction of the foreskin. It may be associated with diabetes, candida fungal infections also.

Do the following investigations:

  1. Blood sugar KOH (potassium hydroxide) mount for candidiasis.

I can suggest you the following treatment:

  1. Gentle retraction of the foreskin daily and soak in warm water to clean the penis and foreskin.

  2. You can use a combination of fusidic acid and Clotrimazole in a cream form, twice daily for one month if balanitis does not respond to topical therapy then you can take Ketoconazole 200 milligrams twice daily for two weeks. Consult your specialist doctor, discuss with them, and take the medicines with their consent.
