Hello doctor,

I am experiencing balanitis. Can you please suggest how to use a cream containing Clotrimazole and Beclomethasone Dipropionate on the affected areas? The affected areas include the head of the penis and under the foreskin covering the head. I am unable to visit a clinic, so kindly suggest some medications.

I read your query and understood your concern.

I have reviewed the picture you attached (attachments removed to protect the patient’s identity). Firstly, I would like to inform you that balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. The predisposing factors include poor hygiene and overwashing, the use of over-the-counter medications, and non-retraction of the foreskin. It may be associated with diabetes and candidal fungal infections. I suggest you undergo investigations including getting checked your blood sugar level and a KOH (potassium hydroxide) procedure for candidiasis. I suggest you follow the below-mentioned instructions:

  1. Gently retract the foreskin daily and soak in warm water to clean the penis and foreskin.
  2. You can use a combination of Fucidic acid and Clotrimazole twice daily in place of the cream you mentioned for one month.

If balanitis does not respond to topical therapy, I suggest you consult a specialist, talk with them, and take the medications with their consent. They might prescribe Ketoconazole 200 mg twice daily for two weeks. I hope this has helped you. Kindly follow up if you have more doubts.
