Hello doctor,

I been having some mild burning when I pee since middle of last month. Later, the inflammation spread to my foreskin and tip of my penis. Now I have pain on the tip of my penis. I tried Clotrimazole 1 week did nothing then after 2 weeks I tried 0.5 % Hydrocortisone, it did not work well. I do not know if what I have is Candida or general inflammation. I have pictures and tests. I had tests after 1 week Clotrimazole was normal.

From history that you gave, it seems like candidal balanitis only. But to make confirmative diagnosis, I need to ask you a few questions. Do you have any discharge from penis opening? If yes, color of discharge yellow or white? Even if it is a very mild discharge. Any history of recent sexual exposure? I think candidal balanitis has become candidal balanoposthitis. that is what causing pain in forskin and penis head. But we must rule out gonorrhea just in case.

I have no discharge, no recent sexual exposure. All tests normal, did you view the tests? I had stinging sensation while urinating then inflammation went to glans and foreskin. I have pictures if you want to examine.

Yes, photos will definitely help because I just I do not want to give treatment only on the basis of history. If it is balanoposthitis, we need to treat it and take precautions because sometimes it may cause phimosis. I saw your test reports (attachment removed to protect patient identity). It is common sometimes to not detect infection in urinary examination. In the test, gonorrhea, chlamydia and yeast are not seen. So, they are ruled out. Balonoposthitis or dermatitis due to some irritant are mostly the cause. Retract your skin a few times daily to prevent phimosis. Clean your glans properly. Do not scrub or over-wash your genitals. Take tablet Fluconazole 150 mg for 7 days twice daily. Only two tablets, mix and apply: Cotrimazole cream and Hydrocortisone (in very small quantity once daily for 2 weeks). Stop Hydrocortisone after 1 week. Sodium Fusidate cream two times daily for 7 days.
