I have a small colored patch on the inner side of the calf that has not changed for over five years. Please help.

I read your query and understand your concern. The attached image (attachment removed to protect the patient’s identity) shows inflammation with scaling. It appears to be dyshidrotic eczema. Possible differential diagnosis could be maculopapular drug eruption or annular inflammatory lesion due to autoimmune skin response to antigen (can be an endogenous factor involved-atopic factor) or exogenous component (irritant-insect bite, systemic antibiotic or trigger or provoking factors). Apply Clobetasol propionate (a highly potent steroid) at night for two weeks to reduce the redness. Apply Tacroz (Tacrolimus) 0.1 percent gel in the daytime for 10 days, after stopping the steroid. Apply Physiogel hypoallergenic lotion immediately after bath for days two weeks. Do not scratch or rub the lesion. Keep checking on sugar levels. Avoid known triggers. Increase vitamin C intake in your diet. Kindly answer the following questions:

1. Did you have any vascular disorder previously?

2. Are you on certain medications?

3. Do you have contact with irritant chemicals or lotion?

4. Are you using tanning beds?

5. Do you have prolonged exposure to the sun?

6. Do you have any history of photosensitivity?

7. What is your occupation?

8. Did you have a fever?

Hello doctor,

Thank you for your detailed response. I think it comes from a mosquito bite a long time ago. I do want to add that it feels a little hard when you squeeze at it and sometimes transparent liquid will come out when you squeeze. Does it fit with your diagnosis?

I read your query and understand your concern. It is post-insect bite eczematoid dermatitis as per your explanation. Apply Fucibet cream (Fusidic acid plus Betamethasone valerate) which is a combination of topical antibiotic and potent steroid. It will be working for you. Do not scratch or rub the lesion, it may cause secondary bacterial infection. As the symptoms you are presenting clinically, could be signs of melanoma (skin cancer). So observation of lesions is needed. Observe the change in the size of the lesion, the border or margins of the lesion, and the color of the lesion. Revert back if any of these changes are found. Avoid prolonged exposure to the sun. Wear sunscreen lotion. Use sun protection measures. seek shade whenever possible. Avoid the use of tanning beds. I hope I have cleared all your doubts.
