Hi doctor,
I have combination skin with blackheads and whiteheads. I also have tanning on my face and neck, mild acne at times, uneven skin tone, and acne marks. What to do?

Combination skin with blackheads, whiteheads, mild acne, uneven skin tone, tanning, and post-acne marks is very common, especially in patients with a history of acne and underlying PCOS (polycystic ovary syndrome), as hormonal imbalance can contribute to recurrent breakouts and pigmentation.
Your concerns can be managed effectively with a simple, consistent skincare routine and appropriate medications.
For treatment, apply Azelaic acid 20 percent daily at bedtime in a thin layer over the face and neck. Azelaic acid helps reduce blackheads and whiteheads, control mild acne, improve uneven skin tone, and lighten tanning and acne marks. Mild tingling or dryness may occur initially, which usually settles with regular use.
In the morning, apply Clindamycin gel as a thin layer over acne-prone areas. This helps control acne-causing bacteria and reduces inflammation. After this, use a broad-spectrum sunscreen every morning, even if you are indoors.
Sunscreen is extremely important to prevent further tanning, pigmentation, and darkening of acne marks, especially when using active creams.
For cleansing, use face wash twice daily to gently remove excess oil and impurities without stripping the skin. Follow this with moisturiser, applied both morning and night, to maintain the skin barrier and reduce dryness or irritation caused by acne treatments. Even combination or acne-prone skin requires regular moisturization.
Lifestyle measures also play an important role. Avoid touching or picking at acne lesions, as this can worsen marks and uneven texture.
Try to limit excessive sugary and junk foods, manage stress, and follow your PCOS treatment regularly, as hormonal control helps improve long-term skin outcomes. Avoid harsh scrubs, frequent facials, or home remedies that may irritate the skin and worsen pigmentation.
With regular use of this routine, you should begin to notice gradual improvement in acne, skin texture, and skin tone within six to eight weeks.
