Hello doctor,

I had sex with my boyfriend twice two weeks before. It was protected sex and my last period was five days before that. After a week, I started a yoga teacher training course where we did intense yoga for two hours, and today I spotted light bleeding in the morning. I am stressed. Does this happen because of heavy intense yoga? I have never done this kind of intense workout in my life. Or is it something related to pregnancy, but it was protected sex? And I feel UTI infection also sometimes. I have itching in my private part. I am worried.

To get pregnant, the sperms have to be released inside the vagina of the female. These sperms then need to fertilize an egg which may or may not be available depending on the ovulation pattern of the female. The fertilised egg is then required to be implanted in the uterus of the female. When all this happens successfully, a female is said to have conceived, and she may or may not experience light bleeding, commonly known as implantation bleeding. Now in your case, the intercourse was protected, so no sperms were released unless the condom was bad or ruptured or, in other words, a contraceptive failure.

Further, the intercourse was within ten days of periods, so at least for the first intercourse, you were not ovulating and, similarly, maybe for the second one as well. So there is no egg in all likelihood. So do not worry about the pregnancy part.

It is too early to do a pregnancy test, so maybe if you want, you may do a urine pregnancy test at home with the first urine of the day. Now coming to bleeding, yes, intense workout or exercise sometimes leads to small bleeding. Coming to the next point of itching, please tell me if you are also experiencing any unusual discharge or odor in the vagina. Do not worry; I am always there for you.

No, I do not feel any unusual discharge or odor. Little discharge always come from my vagina all month since my teenage. Pregnancy is 99.9 percent not there, as explained above. Bleeding happens in some cases due to intense workouts. For the itching part, with no discharge and no burning sensation while urination, I suggest:

  1. Use a good feminine wash to clean the area.
  2. Keep the area dry.
  3. Wear loose cotton underwear.
  4. Observe excellent hygiene.
  5. Use a soft and gentle detergent to wash underwear.

If your itching does not improve, then I will suggest ointment or creams.

How is your yoga going? If you read our previous conversation, we understand that for pregnancy to occur, sperm and egg have to meet. As I understand, you generally have periods without any irregularities. So, this time also, if we go by that, then you were really not ovulating around this month, and most likely, an egg would not have been there. But regarding the fact that your boyfriend did not ejaculate inside, even precum has a significant sperm content to cause pregnancy. Ideally, you should have taken emergency contraceptive pills immediately, but now that option does not exist, as we are already beyond the time period when these pills are effective.

Now, just wait for around 10 days, and if periods do not come, then we will do a urine pregnancy test at home. Based on the outcome, we will decide what to do next. Most likely, you should not have been ovulating and hence must not be pregnant. Just do not stress, and let me know after 10 days or onset of periods, whichever is earlier.

See, for an intimacy in the middle of this month, the test should ideally be done around the end of this month for a somewhat reliable result. Even if someone has conceived, the test will not come back positive within a week of intimacy. At minimum, the test should be done around the end of this month. Please do not worry. Firstly, stop worrying so much. The test you got done today is a urine routine examination and not for pregnancy. For pregnancy, hCG (human chorionic gonadotropin) is measured in urine, which is not indicated in the report. hCG can also be measured in blood. hCG is a hormone that increases significantly after 10 to 14 days of conception, or roughly after intercourse. It is too early for you to take these tests right now. Nevertheless, there are still some parameters in your report that are slightly on the higher side. Pus cells are elevated. Are you otherwise feeling perfectly fine now? Please let me know.

Nothing to worry about. Have you observed any spotting or very light bleeding until now since the middle of this month? Furthermore, stay cheerful. Even if you would have conceived, you are an adult to decide your life. Eating or not eating is fine as we are doing the HCG test in the smallest time gap possible. Still, just have a word with a possible lab or sample collection facility, how early they can collect and provide the report. If it is in the first half, have a light breakfast and avoid high-sugar content items like sweets. Do not worry. There is no need to have anxiety; you are perfectly fine.

I have gone through the report (attachment removed to protect the patient’s identity), and as you would have also seen, it is below the reference value for pregnancy. So please, now do not stress and be cheerful. What I would recommend, just to be sure, is that we do a urine pregnancy test at home after fifth of next month. Also, my strong recommendation is not to engage in unprotected intercourse, even if it is for five seconds. If you are sexually active, you may consider switching to regular birth control pills (not emergency contraceptive pills). Take care and stay safe.
