Patient's Query

Hello doctor,

I am getting blood from my mouth, and it is not from my gums or coughing. This bleeding is always associated with pain and gasping. What could be the reason for my issue?

Thanks.

Answered by Dr. Prathish Kumar

Hi,

Welcome to icliniq.com.

As per your description, this person is on Aspirin. Aspirin can cause ulcers in the stomach and mouth. The ulcers formed in the stomach can bleed, which will, in turn, irritate the stomach and comes up to the mouth, and during this instance, if some amount of blood enters into the respiratory tract down into the lungs can cause breathing difficulty. The other way around, per se, there can be bleeding into the lungs to cause gasping for breath, and this blood can come up to the mouth and cough out, which is called hemoptysis.

It will be better if you can give us more information for better judgment of the cause.

1) Who is the patient?

2) What is the age of the patient?

3) What has precisely happened for that patient?

4) Whether the patient is alcoholic or addicted to any drugs?

5) Is there bleeding from the mouth itself or coming up from inside somewhere?

Regards.

Patient's Query

Hello doctor,

Thank you so much for the clarification.

It is me, the patient. I am 63 years old. I have had a chest illness for some years. I do not take alcohol or any other drugs. I was getting this off and on for the past three years. It was spontaneous bleeding and not anywhere else; it did not taste like blood but looked like it. The pain was lower down the throat. I had ulcers when I was young.

Thanks.

Hi,

Welcome back to icliniq.com.

If you had got admitted in hospital, doctors would have evaluated for this. So, what did they tell as the cause for your problem? If you have discharge summary, you can share it here so that I can look into it to see if any information is there.

I hope this helps.

Patient's Query

Hello doctor,

Thanks for the reply.

The doctors did not tell me anything. For example, during three days of hospital admission, I failed a treadmill test. I was not able to do at all. But the doctors told me that I had passed it in flying colours and I knew it was not true. I got my medical record a year later, with great difficulty, and it has the truth I did not manage the stress test. They did not give discharge papers but told me I was completely normal.

Please help.

Hello,

Welcome back to icliniq.com.

You are too much stressed out. So, kindly relax. Final conclusion is that, since you are a cardiac patient and now having shortness of breath, you need to visit a cardiologist and get your heart status evaluated at present to look for any possible deterioration. And for your gastric ulcer, you need to continue the Omeprazole tablet twice daily half an hour before food to reduce gastric acid secretion and avoid hot and spicy foods. Add syrup Sucralfate 10 ml thrice daily one hour before your meals along with your Omeprazole for ulcer healing.

I hope this helps.

Thanks and regards.

Patient's Query

Hello doctor,

Thank you for the reply.

I am stressed out about unanswered questions. I had that bleeding from the throat throughout an year and never again. I will note the additional medicine you have suggested and try to get it.

I forgot to tell you that I have H.pylori infection and ulcers.

Thanks.

Answered by Dr. Prathish Kumar

Hello,

Welcome back to icliniq.com.

H.pylori (Helicobacter pylori) infection is an important cause of persistent gastric ulcers. So, it becomes an additional factor along with Aspirin to make your ulcers bleed. And pulmonary embolism (PE) is another important cause of chest pain and breathing difficulty particularly when it is associated with hemoptysis. PE is something similar to MI (heart attack). MI is formation of blood clot and blockage of blood flow in blood vessels supplying the heart muscles. PE is a similar thing that occurs in blood vessels going to the lungs.

H.pylori infection needs a set of antibiotics to be taken for two weeks.

I hope you understood and got your answers.

Thanks.
