Patient's Query

Hello doctor,

My father, aged 55, experienced high blood pressure at 181/94 today and felt unwell. He does not regularly take blood pressure medication. After taking Omazest 20, his blood pressure lowered to 166/94 within two hours. He is also diabetic and takes a fixed-dose combination of Linagliptin and Metformin hydrochloride tablets. He had a restless night yesterday with frequent urination and signs of sleep apnea (though not diagnosed officially, as he snores loudly and breathes heavily while sleeping with an open mouth; I have provided an elevated pillow, which seems to have improved his sleep, but occasional snoring is still noticeable). He also has an enlarged prostate.

I would like to know if my father can take more than one tablet of Omazest 20 to reduce his blood pressure. Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

Sleep apnea is a known cause of resistant hypertension. Identifying the underlying cause and addressing it is important. For individuals who are overweight, weight reduction is recommended. If there are specific issues like tonsillitis affecting the neck, seeking treatment is essential. Consider visiting an ENT (eye, nose, and throat )specialist for further evaluation.

When sleeping, try lying on the right or left side. If sleeping on your back, place a pillow below the neck rather than under the head. In cases where sleep apnea is diagnosed, continuous positive airway pressure (CPAP) therapy can be beneficial. Maintaining a low-salt diet can also contribute to normalizing blood pressure. If blood pressure remains elevated, medication dosage adjustments may be necessary, but this should only be done under medical supervision. Additionally, control blood sugar levels strictly. Regular monitoring of renal function and electrolyte levels is recommended to manage overall health

I hope this information will help you.

Thank you.

Patient's Query

Hello doctor,

Thank you for your reply.

Today, his blood pressure was checked at 144/74, and he is feeling well. Regarding sleep apnea, which specialist should I consult? Are there alternative treatments apart from CPAP? He is concerned about the effectiveness and risks associated with CPAP therapy.

He is actively working on weight loss and I have urged him to schedule a doctor's appointment for a thorough checkup.

I forgot to mention, he also experiences constipation issues and has been prescribed PregRed powder by the doctor. He takes it daily, but there is still some discomfort. Occasionally, he notices blood in the stool (likely hemorrhoids) when not using PregRed powder. What recommendations do you have for this?

Kindly suggest.

Thank you.

Hi,

Welcome back to icliniq.com.

I can understand your concern.

For snoring or obstructive sleep apnea, take him to an ENT doctor. He will examine the throat, and soft palate tonsils to see if anything can be managed. Weight loss is very important. For constipation, I would suggest the consumption of more fiber in his diet and avoiding processed foods.

I hope this information will help you.

Thank you.

I would like to know if there are any other methods for treating sleep apnea other than CPAP. Kindly suggest.

Hi,

Welcome back to iclinq.com.

Additionally, I have suggested the following measures:

  1. Placing a pillow under the neck while sleeping

  2. Trying to sleep on the side rather than on the back

  3. Focusing on weight reduction as part of managing overall health
