Patient's Query

Hello doctor,

My 18-month-old child underwent some routine blood tests. Her results are as follows:

Test result bio ref range:

  1. Vitamin B12 1214 pg/mL (220 – 920 pg/mL)

  2. Aspartate aminotransferase ST (SGOT-serum glutamic-oxaloacetic transaminase) 54 U/L (0 – 40 U/L)

  3. Calcium 10.6 mg/dl (8.5 – 10.5 mg/dL)

  4. Vitamin D 26.3 ng/ml (25 – 100 ng/mL)

Should I be worried or are these values normal for an 18-month-old child? I am not giving her vitamin D drops for one month. Can I give her those?

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understand your concern.

We need to see the context or symptoms for which the tests are done. Does she suffer from any illness? and why were these tests done? Please send details of the patient's condition.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

My child has a beta thalassemia trait. Her weight is 22 lbs. Actually, I wanted to know her vitamin D levels but there was an offer, a package of three tests so I went for that along with the other routine blood work. She has no symptoms, she is active and well. She had been using calcium and vitamin B12 syrup (she is not taking both for the last 20 days) because there was a little developmental delay (gross motor delay which is resolved now) and hence the doctor prescribed it. The following are her blood test results:

  1. Hemogram hemoglobin levels are low (9.3 gm/dL).

  2. RBC (red blood corpuscles) is high (5.3 million/mm3).

  3. WBC (white blood cells) are normal (6800 cells/mm3).

  4. Differential counts are normal.

  5. Absolute leukocyte counts are within normal limits.

  6. Platelets are 4.4 lakh cells/mm3.

  7. The reticulocyte count is 1.3 %.

Peripheral Smear:

  1. RBC: Microcytic, Hypochromic, anisocytosis

  2. WBC: Normal in morphology, maturity, and distribution. No abnormal cells were seen.

  3. Platelets: Adequate.

  4. CRP is 1.0 mg/L.

  5. Random Blood glucose: 120 mg/dl.

  6. Ferritin: 35 ng/ml.

  7. Creatinine: 0.3 mg/dL.

  8. LFT (Liver function test) normal except AST/SGOT: 54 U/L.

  9. Vitamin B12 is 1214 pg/mL.

  10. Calcium is 10.6 mg/dL.

  11. Vitamin D is 26.3 ng/mL.

Kindly suggest the best course of action doctor, I would highly appreciate that.

Thank you.

Hello,

Welcome back to icliniq.com.

I received the reports and they seem pretty normal. (attachments removed to protect the patient's identity). You should give your daughter calcium syrup and iron supplements. Continue breastfeeding and a proper diet.

I hope I have answered your question.

Let me know if I can assist you further.

Regards.
