What causes painful breast lumps with a whitish discharge?

Hi doctor,

Please suggest treatment for the following complaints, such as pain in both sides of the breast on pressure, a small cyst-like structure with whitish discharge through the breast for one month. I have been on past medication Evion 400 mg once daily, tablet Ultracet twice daily, Zevit capsule once daily, and Comfort 0.5 mg one tablet a week.

I understand your concern.

I do not see any reason for you to worry about it. As per the ultrasound report, both breasts and lymph nodes are normal. Let me explain each of your symptoms with possible causes.

1. As you say, with pressure, something is profound in the breast. This is called costochondritis infection in the muscles and deep joints of the chest. It can happen if you repeatedly keep checking or have a recent history of severe cold or upper respiratory infection. If you take a deep breath, you will have that pain at the end.

2. Cyst-like structures at this age are usually the fat accumulation in the breast. Your breast starts sagging at this age. Also, the glands in the breasts that produce milk are no longer active and get clumped to one side, also around the nipple.

3. How many years back did you stop breastfeeding? Repeated breast examination or checking if there is any milk, squeezing the breast frequently will stimulate milk production & brain will recognize this as baby sucking for milk

For treatment, I suggest you:

1. For costochondritis, a good course of antibiotics and painkillers will work.

2. Cyst-like masses do not need treatment.

3. Stop repeated examination of the breast and continue tablet Comfort (Diclofenac).

To rule out other causes:

  1. Get a thyroid profile and serum prolactin test done on an empty stomach in the early morning.
  2. Do a mammogram (X-ray) to rule out any deeper masses.
  3. Continue Evion (Vitamin E).
  4. Have Ultracet (Tramadol) only if you have pain.
  5. You can also add a tablet of Primosa 1000 mg once a day for a month.
