How to manage a lung infection after an open bypass surgery?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My father is 60 years old. Last month, he had some breathing problem and so, we hospitalized him to the nearest available hospital.

In the hospital, we came to know from the doctors that he had a triple artery blockage. The doctors advised for a bypass surgery as his condition was very critical. He had a bypass operation the next day. He was discharged from the hospital about a week back. He was at home for four days.

Two days back, he had breathing issues and his sugar was very low as per the doctors. We hospitalized him in the same hospital and now they are saying that my father is having a lung infection. My father has had type 2 diabetes for the past six to seven years. Now the doctors are saying that his hemoglobin is very low at 8.5 and there is a presence of creatinine traces in blood along with kidney issues.

I have attached his discharge reports. My question is what is the treatment that is required right now? Should we shift him to any other hospital? He is having weakness and his health is also not stable. Please advise.

Thank you.

Hi,

Welcome to icliniq.com.

Kindly share the medical reports again with the latest kidney parameters and the discharge summary along with treatment papers. As per the history reported by you, chest infection is one of the most common complications after a bypass surgery. The presence of morbidities like diabetes and deteriorated heart functions add more to the problem.

As per the history narrated by you, it is most likely pneumonia. His hemoglobin is low. A hemoglobin level of less than 7 gm/dL will require a blood transfusion. Any internal bleeding from the site of the operation has to be ruled out. Do not shift your father anywhere as the treatment pattern would be the same in every hospital.

The weakness and unstable health are mainly because of the ongoing chest infection. Before making any further comment, I would like to see the 2-D echogram reports (before and after surgery), a photo of his latest x-ray done in the hospital, blood reports, blood pressure and pulse right now, treatment chart which the hospital is following, kidney parameter, blood sugar reports and hemoglobin before surgery. Is he admitted in a multi-specialty hospital with all services under a single roof? Advice can be given only after thorough analysis.

I hope that you get your answer.

Please let me know if you need any help.

Thank you.
