I am a 22-year-old male with a weight of 69 kilograms and a height of six feet. I have five large bumps under the skin along the jawline. I have soreness with pain. What to do?

I have gone through the picture (attachment removed to protect patient identity). You have a fungal infection. Tinea barbae is a dermatophytic fungal infection affecting the skin present with coarse hairs such as beard and mustache. Please mention some details about the profession and mention whether you had any recent contact with animals or pets. You can do a micro-examination of a skin scraping. I suggest you consult a general practitioner soon. You can take oral antifungals to treat it.

The Probable causes

You have a dermatophyte fungus and folliculitis.

Investigations to be done

You can do a microscopic examination and culture of a skin scraping.

Differential diagnosis

You can have tinea barbae or bacterial or candidal folliculitis.

Probable diagnosis

You can have a tinea barbae.

Treatment plan

Take tablet Itraconazole 250 mg once daily for four weeks. You can do shaving or hair depilation with warm compresses to remove crusts and debris.

Preventive measures

Avoid having contact with animals or pets.

Regarding follow up

Have a follow up after four weeks.
