Hi doctor,

I recently underwent circumcision. After five to six weeks, my morning erections and even normal erections, especially after masturbation, cause my penis to swell up completely red and cause pain for some time. To reduce the pain, I sometimes take either Ultracet-D or Chymoral Forte as prescribed after my circumcision. However, is it normal for the penis to cause such pain and redness with such high swelling? Also, I have an issue with prone masturbation, which is also causing problems.

I can certainly relate to your worries, and will do my best to assist.

Generally, after circumcision, things settle down in four to six weeks, and there should not be any swelling. However, I suspect that your prone masturbation is causing this issue because your suture line would be rubbing against your bedsheet. I would advise you to stop prone masturbation as it can lead to serious damage to your penile musculature. Normal masturbation is alright as long as you are not too aggressive with it. My only concern is the swelling during early morning erections. Please help me with a few details:

  1. Does this swelling happen with every erection that you have? (Even when you have not masturbated)
  2. Does it hurt when you have an erection (even without masturbating)?

Please share a few photos with me of your penis in both the non-erect and erect state so that I can understand what i s going on.

Thank you for your prompt reply.

Usually, the swelling occurs during masturbation. The redness, however, is always there even if I do not ejaculate. Also, the main problem arises after ejaculation, as it takes many hours for the redness and swelling to reduce or subside after ejaculation. If I do not masturbate, then for weeks, it remains usually normal. My circumcision, which was done traditionally as an open circumcision, took some time for the stitches to dissolve. I have attached a picture when the penis is not erect. I will share another picture too.

I am sorry to say, but in my opinion, I do not think the circumcision is complete. There is still a lot of excess skin over your glans. If you search for a few photos of a circumcised penis, you may be able to see the difference. So, my guess would be that the pain and swelling might be due to the tight skin still present over the base of your glans. The other reason for the pain could be the prone masturbation. I would strongly recommend that you refrain from doing that. I would suggest speaking with your urologist about the circumcision. Is there anything else I can help you with?

  1. Pain medication: Only when needed.
  2. Swelling medication, Chymoral Forte (a combination of trypsin and chymotrypsin): Yes.

As I mentioned before, I suggest that you visit your urologist for a check-up because, based on the photos you have uploaded (and not a physical examination), there seems to be quite a bit of excess skin around your glans, which ideally should be removed.
