Hello doctor,

I am a 64-year-old female. I have an awful cracked, blistered, and swollen foot. I have been going to hot springs and mineral pools for the last three weeks pretty much daily and had itchiness. But in the last few days, it blistered, the skin cracked and wept, and the swelling has affected the foot up to the ankle. I tried tea tree oil ointment and soap as well as lavender, thinking it might be a fungus. I was also sunning. But no improvement. I popped a blister, and the liquid was brownish but clear and did not smell foul. The rash is confined to between the middle three toes. It looks somewhat symmetrical on the top, with weepy blisters on the second and fourth toes, both next to the middle toe. Underneath, the footpad the middle three toes are swollen and itchy. The small and big toes are swollen and painful but not cracked, blistery, or weepy. Please help.

Apply Fucibet cream (Fusidic acid and Betamethasone) at night for 15 days, Mupirocin ointment at daytime, and noon for ten days. Take tablet Zifi (Cefixime) 200 mg twice daily after food for five days, or tablet Ciplox (Ciprofloxacin) 500 mg once in a day after food for five days, and tablet Emanzen D (Serratiopeptidase and Diclofenac) twice daily for three days (if pain). Apply Moisturex soft cream in the evening for ten days. Are you a diabetic? Is there a history of fungal infection in the past? Any history of atopy or allergies? Is there any injury to the foot, foot dermatitis, or eczema? Do you have a recurrent episode?

My feet periodically get mealy skin and crack under the toes, but I have never had anything like this. When I was getting a pedicure several years ago, the technician suggested it might be fungus, but I told her it was just how my feet were in the winter when they got sweaty in the shoes.

Can you use U lac cream twice daily for 15 days, skin first moisturizing lotion after bath for 15 days, and Fucidin cream twice daily for seven days. These can be used in addition to the above-mentioned treatment. Wear comfortable, well-fitting shoes. Avoid friction or pressure on the foot. Wear cotton socks. Maintain proper foot hygiene. Clean area with Povidone-iodine, cotton swab, and water. Daily dressing to avoid secondary bacterial infection. Maintain sugar levels under control.
