Hello doctor,
My boyfriend has noticed, within the last few weeks, a new curvature in his penis when erect (there is no pain or lumps and bumps). We cannot think of any injuries recently, though there was an episode around a year ago when he had some discomfort during sex when we shifted position. We are going to keep a close eye on this, but is a new curvature in adulthood always Peyronie's disease, or can it sometimes be something that is unrelated and does not change?

A new curvature is not always Peyronie's disease. It can be unrelated, minor, and non-progressive. But since Peyronie disease (PD) often starts subtly, keeping an eye on progression and getting checked if anything changes is the safest approach.
It is caused by scarring (fibrous plaque) in the tunica albuginea of the penis. It typically presents with a palpable plaque, pain during erection, and progressive curvature over months. If you have only curvature without pain, probably the disease is no longer in the active phase.
Other possible explanations include natural variation, as some men only become aware of a mild natural bend when they look closely, and not all curvatures are pathological. Minor trauma, such as the discomfort during sex a year ago, might have caused subtle tissue change that only becomes noticeable now, even without pain or a discrete lump. Vascular or connective tissue changes (age-related elasticity differences) can sometimes create a change in angle without a classic Peyronie plaque.
When you must consult a doctor:
Increasing curvature over time.
Pain with erections.
Palpable hard plaque or lump.
Erectile dysfunction develops along with the bend.
If any of these occur, get yourself physically checked by a nearby urologist.
