Hi doctor,

I had a cystoscopy and retrograde pyelogram three days back, as suggested by a urologist, to help diagnose unexplained microscopic haematuria. And now, I am experiencing post-operative pain or symptoms not mentioned in the post-operative guidance. I will be traveling for 24 hours tomorrow (long haul flights and trains) with limited access to medical care, and I want to confirm if I should be taking any action for my symptoms before I travel. Also, I would like to add that I am drinking approximately 3 to 4 liters of water daily. While voiding, the urine is clear and has no smell with minimal or no stinging sensation. I have a few symptoms, like a constant dull ache in my lower left back or flank, which started after my first void in the hospital. The sensation is like pressure on a bruise. It has not worsened or been better since the surgery three days ago. I rate my pain as two out of ten on a scale of ten. I have constant pain when lying down. And I rate the pain as four out of ten when sitting or standing for more than five minutes. Also, the pain increases to seven out of ten post void if I fully empty my bladder and coincides with bright red blood in the urine (blood only visible at the termination of void).

My GP, hospital, and specialist are not answering the phone today because they are closed. Could you please advise on suggested actions for the above? Does this sound normal and likely to improve over the next few days without intervention?

I went through your query and understood your concern. Cystoscopy retrograde will only improve unless you got a urinary tract infection as a complication from the procedure. If you got DJ (double-J) in addition to cystoscopy retrograde, those symptoms would not improve; in fact, they will worsen and require medications. So I suggest you to do a urine analysis and an ultrasound scan and send me the reports so that I can tell you everything.

Can you please clarify what you mean by "DJ in addition to cystoscopy retrograde"? I had a rigid cystoscopy and a retrograde pyelogram. From your experience, is it normal to have lower back or kidney pain after this procedure?

Cystoscopy and retrograde pyelogram cause temporary post-operative pain for two to three days. If pain persists it means you might be having an infection or you might have a DJ stent placed during the procedure. DJ stent is a silicon tube that can be placed in the ureter after the procedure to optimize the draining from the kidney on a temporary basis like two to three weeks time post-procedure. Sometimes it is placed and sometimes they are not placed. Usually, the surgeon tells you if they placed it.
