What could cause severe pain in a filled molar tooth?

Hello doctor,

I am experiencing severe pain in the first molar tooth. I had a filling done one year back. I had three Combiflam tablets to reduce the pain. Please advise.

I can understand your concern.

As per your description, it seems that there is a reinfection in your treated tooth. Do you see any discoloration in that tooth? For a permanent solution, you have to get it treated in a dental clinic. Reinfection can be confirmed only after taking an X-ray of that tooth. But as per the current outside scenario of a viral outbreak, one should not visit the dental clinic unless it is an emergency. So what you can do at home is take the tablet Augmentin 625 mg (Amoxicillin and Clavulanic acid) twice daily for three days after food, Ultracet P (Tramadol) twice daily for three days after food, and gargle with lukewarm water by adding a pinch of salt to it. Do this for two to three times daily. If possible upload the image of the concerned area.

I need to know a few more details. Is there any swelling or pus associated with that tooth or gums? Is there any reddish discoloration or are you having any fever? I want to know what kind of restoration you had. Kindly answer my queries.

Thank you for the reply.

Yes, there is swelling observed from the outside. I have no fever but my jaw is paining. No discoloration is observed. I am not sure about the kind of restoration. I will check the prescription and revert.

Your images are not that clear (attachments removed to protect the patient's identity). If swelling and inflammation are there, then take the tablet Zerodol SP (Aceclofenac plus Paracetamol plus Serratiopeptidase) twice daily for three days after food instead of Ultracet P (Tramadol and Acetaminophen). Take Ultracet P (Tramadol and Acetaminophen) only if you have severe pain. Also, take Pan 40 mg (Pantoprazole) half an hour before food once daily for three days to prevent acidity. Yes, from this application only, I have written down a prescription. I have prescribed medicines for acidity, infection, and swelling.
