Hello doctor,

I am a 34-year-old male working as an engineer. Regarding my mental state, I feel lethargic and sleepy all day. I do not have an interest in doing anything in life. Even if I start something, I am easily bored. I am always in a hurry. I have rapid mood swings. I cannot concentrate while reading as my mind is restless.

I constantly have negative thoughts. Most of the time, I am sad. I am short-tempered, and my pulse rate increases rapidly. I often want to smash things or throw things at people. I ponder doing crazy things and anticipate the outcomes of doing silly things. I have repetitive thoughts like a maniac.

I do not feel like doing anything. I have lost interest even in my favorite activities. I consulted a psychiatrist, and he diagnosed it as major depressive disorder with OCD traits and hypertension. But, I do not want to take medicines unless I know for sure it is really going to cure me.

Please help.

The kind of thoughts that you describe and the fact that they keep coming repetitively are diagnostic of obsessive-compulsive disorder (OCD). Your complaints of weakness, mood swings, anger, and a loss of interest in doing things are typical of depression. So, your doctor's diagnosis is correct.

I would suggest tablet Fluvoxamine as the first choice of medicine. For OCD and depression, the best and easiest treatment, which will give early results, is medication. Other therapies, like psychotherapy, etc., are not effective when you suffer from a severe type of OCD and depression. So, I believe that they will prolong your suffering. I would suggest you start tablet Fluvoxin CR (Fluvoxamine) 50 mg and Rivotril (Clonazepam) 0.5 mg. Both tablets are to be taken only at night. After one week, the dosage of Fluvoxin CR can be increased to 100 mg while keeping Rivotril the same. Update me regarding your blood pressure readings if you are going to use any anti-hypertensive medicine. Thank you.
