Hello doctor,

I am a 34-year-old male. About four months ago, I was diagnosed with moderate depression. My psychiatrist started me on Seroquel 200 mg at bedtime and gradually increased it to 800 mg at bedtime. Together with Cipralex 20 mg. I feel good improvement. But I noticed that I could not sleep at all if I did not take the Seroquel. I am dependent on this medicine to sleep and noticed significant weight gain of about 15 kg over four months. I belong to a family with a history of diabetes, and I am aware that Seroquel could cause diabetes. I have another concern about delayed and sometimes absent ejaculation since I started this combination (Seroquel and Cipralex). I went back to my psychiatrist, but as per him, I need to continue on both medications until I significantly improve. My question is, do I need to decrease the Seroquel dose as it is 800 mg at night? Is it true that I will be completely dependent on Seroquel to sleep? Is this combination (Seroquel and Cipralex) good for me? I wake up in the middle of the night feeling hungry and need sweets to eat. Is it normal? My parents, cousins, and most of my family members are diabetics. Is this increasing my risk of being diabetic also? Thank you in advance.

I can understand your concern. If you are suffering from depression, then why are you on tablet Seroquel? It is an antipsychotic and mood stabilizer. It may be used if you have resistant depression. If you have the first episode, then you do not need to take tablet Seroquel at all. You can ask your psychiatrist about it. Tablet Seroquel is not a first-line medication for depression. There are many other good antidepressants with minimal side effects. You can taper off your tablet Seroquel gradually. You can take another medication for sleep disturbance like tablet diazepam, which has no side effects like weight gain or diabetes. If you have been on tablet Seroquel for a long time with a positive family history of diabetes, then you need to monitor your sugar level once a year.

You should also follow the below lifestyle for depression. It is nonpharmacological management and will help you. You should do exercises daily for 30 to 45 minutes, like at least going for a walk. If possible, join the gym or go swimming. You should do meditation for 20 minutes daily, just sit at one place in a comfortable position and try to concentrate on your normal breathing, and do not stop or force yourself to stop your thoughts. Just let it be. You should eat healthy home-cooked foods and avoid junk foods. You must drink enough water. You should spend quality time with friends and family daily, or video call them if you are living away from your family. You should spend time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills or language or musical instruments. You should take enough seven to eight hours of sleep daily. You must avoid tobacco or alcohol or any substance abuse. Live your life, do not just spend it. Be bold and confident.
