Hello doctor,

For the past six months, I have had an irritating aggressive dandruff on my right eyebrow. Granules develop that are painful to the touch. I have applied zinc oxide, tea tree oil, and Pramoxine hydrochloride 1 % (suggested by a pharmacist). They helped for a while but were not a permanent solution. Glycerine made it worse. Please give some suggestions.

Is this the same episode or a recurrent issue? How long did you use above mentioned topical agent? Do you have discharge from the right eye? From your description, one of the differential diagnoses is blepharitis. But to be sure of the diagnosis, kindly send the image and revert back. Depending on the severity of inflammation, topical agents can be advised.

Thank you doctor,

As I said, this only involves my eyebrow. It does not involve the eye, eyelid, or lashes. It is not blepharitis. It is one episode that has continued for six months. I have used different ointments at different times. If I scratch it too hard, it bleeds. I have no image to send; you would only see a little redness anyway. Please suggest some topical agents.

My mistake; I thought it involved your eyelid too. Scaly plaque associated with itching over hair involving the skin. Hence, the differential diagnosis can be seborrheic dermatitis or atopic dermatitis. Clinical photographs would have helped to diagnose. Topically, consider the application of a cream containing mild steroids, which would help to reduce inflammation, along with an antifungal cream.

Seborrheic dermatitis treatment is with a combination of steroids and antifungals, like Momate cream (Mometasone Furoate) along with Ketoconazole 2 % cream. Apply twice daily for seven days, then once daily for seven days. Please follow up after 15 days regarding improvement.
