Hello doctor,

I have been dealing with depression, anxiety, and IBS-related diarrhea for the past decade. I have been taking psychiatric medications such as Nexito Forte, Nortimer 25 mg, and Clonazepam to manage my condition. Recently, I have noticed an increased frequency of urination, leg pain, and persistent fatigue. After conducting a diabetes test, I discovered that my fasting blood sugar level was 125, which rose to 170 after meals. My HbA1c level is 7, which is on the higher side. I am concerned about whether I should start taking diabetes medication, as I am worried that it might negatively impact my stomach and worsen my IBS-related diarrhea. Please help.

I have read your query and understand your concern.

It seems like you might have diabetes. But do not worry, we can try a few things first. I suggest you focus on your diet and exercise for the first three months. Cut down on food high in carbs, like rice and sugar, and aim for a moderate-intensity 45-minute walk daily. If, after these three months, the HbA1c (a blood test that measures the average blood sugar levels over the past two to three months) level is still high, then we can consider diabetes treatment. I suggest you try taking Metformin XR 750 mg once a day after lunch. It might initially cause loose stools and upset your stomach, but these side effects usually disappear after a few days of regular use. Consult your doctor and take the medicines.

Thank you for your response.

I feel sexual weakness due to excess masturbation in my childhood. I have a premature ejaculation problem. Also, I have depression, anxiety, and a diarrhea problem for which I am taking psychiatric medicine. Mostly, ayurvedic medicine does not suit me and creates stomach upset and ibs diarrhea.

Could you please suggest overcoming my sexual weakness without affecting my IBS diarrhea?
