Diabetes is a strong risk factor for kidney disease. To confirm whether it is because of diabetes or other factors, a kidney biopsy is confirmatory, but it is not required at present.

  1. What about her urine routine report?
  2. What was her last creatinine level?
  3. Has she got an ultrasound of her kidneys?
  4. Does she have any swelling in her legs?
  5. Any medication she is taking presently?
  6. Any diet she is following?

  1. No ultrasound of the kidneys has been done yet.
  2. There is no swelling in the legs.
  3. Yes, she has been taking heart medication for the last eight years.
  4. There is no diet she is following.

This is a condition known as chronic kidney disease (CKD). Alpha keto analogue, two tablets of 200 mg each, can be given thrice daily. There is no other drug that may prevent or reduce the creatinine level. Only the prevention of dehydration and infection may reduce the progression of CKD. Diet plays a vital role. Non-vegetarian diet should be completely restricted. Fruits and vegetables that she may take are brinjal, bitter gourd, fenugreek, peas, lady finger, capsicum, and jackfruit. Apple, pineapple, grapes, guava, papaya, and pear can be taken as fruits. It is best to use multigrain flour, including ragi, bajra, and sorghum, for chapati. You may apply a little ghee to the chapati to maintain the fat content in your diet. Nepro LP protein powder (Maltodextrin) can be used in two scoops twice daily to help with protein requirements. Four almonds and two walnuts she can take per day. Vegetarian diet itself is very helpful in preventing the progression of kidney disease. The only thing to remember is dehydration, infection, and kidney toxic drugs like painkillers and antibiotics may cause disease progression quickly, so that must be prevented. Hygiene has to be maintained. Blood pressure and sugar are to be controlled strictly. Salt intake should be less than 5 gm per day. Once in 10 days, she may consume food of her choice because restricting food strictly for kidney disease may depress the person.
