Hi doctor,

I am 35 years old and have had adenomyosis for the past seven years after my second delivery. I am having heavy bleeding, and severe pelvic pain. I was put on the tablet Meprate 10 mg twice daily for 21 days for the past year but I have stopped taking them now. Still, my heavy bleeding has not decreased. So I took the tablet Trapic MF during heavy periods. I heard about Dienogest tablets. Can you tell me if it is good for me and how long I can take it, so I do not have to remove my uterus?

I read your query and can understand your concern.

Please send me your detailed gynecological, medical, and surgical history. In addition, if you have a recent MRI (magnetic resonance imaging) or any other investigation, then it would be helpful for me.

Adenomyosis, unfortunately, does not have any treatment. You can have symptomatic treatment from hormones, which will give you temporary relief. You can have contraceptive hormonal pills but try to avoid similar hormones for a long time as they will give you short-term and long-term side effects. Cyclic hormonal contraceptives will give you hormonal support, will try to relieve pain due to hormonal imbalance, if any, and will prevent unscheduled bleeding. Try to have one transvaginal scan or MRI to check the extent of pelvic pathology. Unfortunately, a very long-term contraceptive for adenomyosis could not be given.

If you have completed your family, then just a total abdominal hysterectomy and bilateral salpingo-oophorectomy would be the option for you to get rid of symptoms if other pathology had been excluded.
