Hi doctor,

My daughter has skin irritation near her hands in the joining area. After itching, she gets blood in that area. And at nighttime, she is having itching on her whole body, and she has dry skin. The skin problem has been there for the last 25 days. My daughter is 6 years old, and she is a thyroid patient. I had given Monticope and Cepodem syrup. Currently, she is on Erythromycin and Lecope. Please suggest a solution.

Kindly attach the pictures of the skin problem and of the old prescription. The picture that you have sent is that of a blood-soaked cloth (attachment removed to protect the patient's identity). How much is her thyroid level? Is she taking the tablet Levothyroxine sodium (Eltroxin)? Does she have wheezing or dust allergy? Send me the pictures of the hand and joint for a detailed evaluation.

The skin lesion is looking very infected and is oozing (attachment removed to protect the patient's identity). She is atopic with raised IgE (immunoglobulin E) levels. Atopic conditions involve very itchy and dry skin. Because of scratching, it is totally damaged. I suggest you start her on the following:

  1. Tablet or syrup Prednisolone (Omnacortil) 5 mg or 5 ml twice daily for seven days.
  2. Tablet Augmentin (Amoxicillin and Clavulanic acid) 625 mg half tablet twice daily for one week.
  3. Syrup Monticope (Montelukast and Levocetirizine) 7.5 ml twice a day for two weeks.
  4. Use HH Fudic (Mometasone furoate and Fusidic acid) cream at night and use aloe vera gel (Calosoft-AF) lotion in the morning.
  5. Take NS (normal saline) 100 ml bottle. Soak thin gauze pieces in saline and put over the red skin for 10 minutes before applying cream or lotion. Apply twice a day. Send pictures after two weeks.

She is having severe urticaria rash which is common in atopics. It could have been aggravated due to the summer heat. I suggest you apply aloe vera gel (Calosoft-AF) lotion in the morning and at night and take the tablet or syrup Prednisolone (Omnacortil) 5mg or 5ml twice a day for five days. Continue giving the syrup Monticope (Montelukast and Levocetirizine) 5 ml twice a day for three weeks.

Thank you for the reply.

For how many days should I continue giving this medicine? After stopping the medicine, she got the itching again. The last time when I gave her this medicine, she did not have any itching but after stopping the medicine she got the itching back after a few days. Can you suggest any permanent solution for this? Also, if I give the medicine continuously will there be any side effects? Now, she is not on any medication. I had applied only the cream when she had stomach itching which got better afterward. But now she is having itching on the back, on the shoulders, and sometimes in her hand. Please help.

There is no permanent cure for atopy. With age, there can be an improvement. You need to boost her immune system. There is no problem with continuing the syrup Monticope (Montelukast and Levocetirizine) and using other skin creams. The syrup Prednisolone (Omnacortil) should be used sparingly. Kindly get back before doing the allergy test for finding out the triggers.
