Hello doctor,

I am suffering from dry and peeling skin on my penis head which has a lot of painless deep wrinkles. I am also suffering from premature ejaculation. I have had no history of intercourse in the past one year. How can I reverse this condition? I had a history of jock itch last year.

If there was unprotected sex, you need to think of sexually transmitted infection. I suggest you take the tests mentioned below:

  1. TPHA (treponema pallidum hemagglutination).
  2. HPV (human papillomavirus) DNA (deoxyribonucleic acid) PCR (polymerase chain reaction) test.
  3. HSV (herpes simplex virus)-1 and HSV-2 IgM (immunoglobulin M).
  4. Chlamydia trachomatis IgG (immunoglobulin G).
  5. HIV (human immunodeficiency virus)-1 and HIV-2 antibodies screening tests.
  6. HbsAg (hepatitis B surface antigen).
  7. Urine complete test.

If not, it can be due to a simple bacterial infection. In that case, I suggest you the following medications:

  1. You can use ointment T-Bact (Mupirocin) for local application and tablet Doxy (Doxycycline) 100 mg twice a day for five days.
  2. If you have itching, you need to think of a fungal infection. You can use ointment Candid-B (Clotrimazole) for local application, and for itching, you can take a tablet of Cetzine (Cetirizine) once a night.

Make sure you wash your genitalia twice a day in the morning and at night. Use normal water, and do not use cold or hot water. Do not use a heavy pH soap.

Regarding your early ejaculation, there are many reasons for premature ejaculation, as mentioned below:

  1. Stressful work like night duties and twenty-four-hour jobs.
  2. Diabetes, hypertension, and thyroid issue can affect premature ejaculation.
  3. If you have been on long-term medications for hypertension or anti-psychiatric medications.
  4. Frequent masturbation or prone masturbation.
  5. Watching porn frequently.
  6. Receiving less foreplay from a partner or lack of knowledge regarding foreplay and female masturbation.
  7. Having an affair that gives you a guilty feeling.
  8. Lack of exercise or obesity.

You need to check FBS (fasting blood sugar) and PPBS (postprandial blood sugar) to rule out diabetes. You need to check your BP (blood pressure) and free T3 (triiodothyronine), T4 (thyroxine), and TSH (thyroid-stimulating hormone) to rule out thyroid issues. You can use the tablet Duralast (Dapoxetine) 30 mg as a temporary solution. There is a natural method as well. To increase the foreplay duration during sex, women should receive 15 to 20 minutes of foreplay, and men should receive 10 to 15 minutes of foreplay for a good blood supply to the penis for a long and hard erection. Foreplay should be in a relaxed manner. Your partner should learn to give foreplay in a way that you do not ejaculate but enjoy. You can try relaxing music. Delay condoms can help you. Inserting the penis into the vagina is not everything in sex. Sex is not a race, and a long duration does not mean your partner is happy. Love your partner, learn about female masturbation, and give your partner a good orgasm or climax.

Hello doctor,

Thanks for the reply. As I have written above, there is no intercourse history in the past, and there was no itching. I have been using the medicine you recommended, T-Bact ointment, for three days, but I do not see any difference. There are lots of wrinkles, and the skin is discolored and dry with skin peeling. So what should I do now? In the past, I had lots of dry masturbation. I have also used coconut and castor oil for wrinkles and dry skin, but the condition is the same. Please help.
