Hi doctor,

I would like to ask about my friend's condition. She is 27 years old, with a history of eczema. She has papules and blister-like lesions on her chest, hand(resolved), and currently lower limb. Watery discharge and itchy as well. She is working as a pharmacist, she tried Hydrocortisone cream, but it keeps recurring. She denies the blisters getting worsened after stopping Beclomethasone cream (used once or twice a week) and denies shaving her hair. Her fasting blood sugar is normal.

Kindly give your advice on this.

I have seen your query and understand your concern.

I have examined your friend's picture (attachment removed to protect the patient's identity). It seems she has dyshidrotic dermatitis (Podopompholyx). Podopompholyx generally causes itching of the hands and feet, because usually hands and feet directly come in contact with external objects like pollen, dust, detergent, etc. on a day-to-day basis. The substances that aggravate this condition are pollen, chemicals in detergents, paints, solvents, and rubber materials. When she has an allergy, she should stay away from the allergens. There are chances of blisters being infected if you puncture them so do not puncture them.

To control itching, I would suggest your friend to start taking tablet Cetirizine and topical steroids like 0.1% Mometasone twice daily for one week.
