Can a low-voltage ECG be a normal variant?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

The electrocardiogram (ECG) shows low voltage with a QRS of 86 mm. I am physically fit and active, and I did not have any such issues a year ago. What could be the cause, and what is the solution?

Please help.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

Could you upload your electrocardiogram (ECG) image? It should show 86 msec. A low-voltage ECG can be a normal variant and may also be seen in obese individuals, smokers, or patients with heart conditions such as fluid around the heart or lung disease. However, if you do not have any symptoms, it is likely a normal variant. Patients with a condition typically experience other symptoms as well.

Thank you for consulting me.

Patient's Query

Hi doctor,

Thank you for the reply.

Yes, it is 86. I have uploaded my electrocardiogram (ECG).

Kindly advise.

Hi,

Welcome back to icliniq.com.

The electrocardiogram (attachments removed to protect the patient’s identity) is fine, and there is nothing to worry about regarding the electrocardiogram. What is your weight?

Thanks and regards.

Patient's Query

Hi doctor,

Thank you for the reply.

I am not obese, a non-smoker, and I have no history of heart issues. I weigh 163 pounds.

Hi,

Welcome back to icliniq.com.

The electrocardiogram is fine. Low voltage is seen only in some peripheral leads, so this level of variation is normal. It does not suggest any specific abnormalities.

Thank you for consulting me.
