Hello doctor,

I have got an itchy rash and it has become dry now. The itching is worse in heavy sun exposure. It is getting cracked now. How to treat it? Please help.

I have gone through the details and image provided from your end (attachment removed to protect patient identity) and I do understand your concern.

The concern you are having is Eczema. Eczema is an allergic reaction to the skin that causes thickening, drying, and cracking of the skin. They may be due to an external or internal stimuli. External stimuli include exposure to the sun, exposure to cold or dry weather, exposure to any allergic fabrics, paints, nail polishes, creams that dry skin up, medications like vitamin A or anti-anxiety, or any depressive to acne medications due to food items which increases body temperature like hot and spicy food, meat, alcohol and so on. Internal stimuli include changes in body temperature, stress, fever, sometimes sudden temperature changes after bathing due to genetic susceptibility, hormonal fluctuations, etc. They may appear as dry, thick areas on the skin and may appear on any part of the skin. They also may be extremely itchy and sometimes painful. There may be occasional oozing of a clear discharge, or sometimes wounds may form and bleed. Eczema can be managed with medications but may appear on and off whenever there are exposures to the mentioned stimuli as well.

I suggest you the following:

1. Logifeel cream (Glycolic acid and Urea cream): Use this cream as a regular moisturizer on the affected area once or twice daily. The idea is to keep the body's skin extremely soft and moisturized.

2. Lozivate MF (Clobetasol and Salicylic acid) ointment: Use a very thin layer of this ointment on the affected area at night, keep it overnight, and wash off in the morning. Continue for two to three weeks.

3. Tablet Levocet M (Levocetirizine and Montelukast): Consume one tablet 2 hours before bedtime daily for three weeks to help with itching.

4. Capsule Evion 400 (Vitamin E): Consume daily after dinner for two months. They help with hydration and skin moisturization.

5. Capsule Seacod cod liver oil capsule: Consume one capsule daily after dinner for two months. Helps with the hydration and moisturization of skin.

6. Coconut oil massage before bathing once in two days may be considered. Massage with normal temperature coconut oil for 10 mins, keep on for 1 hour, and wash off with soap.

7. Oilatum soap: Use this soap for bathing.

8. Lukewarm salt water and 1 tablespoon of coconut oil may be mixed in a tumbler or vessel and soaking of the affected area or application of the mixture on the affected area with a cotton for 10 to 15 minutes is advised.

9. Mupi ointment (Mupirocin): This ointment may be used thrice daily on open wounds to prevent secondary bacterial infections.

Always make sure to keep skin extremely soft and moisturized. Do not use harsh chemicals or creams. Do not use hard fabric. Avoid scratching as much as possible.
