Hello doctor,

I am a 37-year-old male and weigh 143 pounds. Three months ago, the pain started after masturbation in my right side testicle area, painful areas are the scrotum, groin, and right side upper thigh near the scrotum. The same type of pain happened five years ago and took two months to resolve. The pain is still there. The urologist first prescribed me Levofloxacin 250 mg one time and Nimuslide 100 mg two times for ten days, but the pain did not decrease. Then consulting the second time the urology doctor, he prescribed Levofloxacin 500 mg one time, a tablet Dynapar two times for ten days and a scrotum ultrasound. After completing the course, pain is reduced to 50 percent but not gone. There is a mass-like feel below the left testicle when I self-examined it, and it shows in ultrasound also. All blood, urine, and sugar reports were normal. Give me advice on what is that bump-like thing below the left testicle. There is no such feeling in the right testicle or below it.

I read your query and can understand your concern. Sorry to hear about your trouble. From the report and what you are describing, it appears you have epididymitis on the left side. Please help me with a few more details: Are you diabetic? Do you have any difficulty in passing urine - like straining to pee? Have you had any surgeries in the past or a Foley catheter inserted? Have you had any sexually transmitted infections in the past? Do you have any discharge from the meatus? I suggest you follow some preventive measures: Please wear a tight underwear for two weeks. A size smaller than what you usually wear. I suggest you take tablet Doxycycline 100 mg twice a day for 21 days, and tablet Chymoral forte one tablet thrice a day for 14 days.

Thanks for the reply.

No, I am not diabetic. There is no difficulty in passing urine. I had surgery of anal fistula five years ago but catheter was not inserted. No discharge from the meatus, but yellowish discharge happened with pain about six years ago. That time I took medicine from a urologist, which was resolved after medication.
