Hello doctor,

Approximately nine months ago, I received a diagnosis of epididymo-orchitis and a small varicocele on the left side of my scrotum. My doctor prescribed various antibiotics, successfully treating the epididymo-orchitis, as confirmed by a urinary tract infection test. Despite diligent treatment and the use of Ibuprofen to address inflammation, the left side of my scrotum still presents a discernible mass, causing frustration. In contrast, the right side feels normal, devoid of noticeable swelling or mass. My inquiry revolves around the potential subsiding of the epididymal enlargement. I am eager to know if, after overcoming epididymo-orchitis, the left side of my scrotum can eventually regain the same sensation it had before these conditions arose. I am interested in understanding if it is possible to eliminate the sensation of swelling and mass on the left side of my scrotum, aiming for a similar feel to the right side. Please help me.

The swelling of epididymo-orchitis does take a long time to resolve in everyone, even after the pain has subsided. You will need to undergo a color Doppler of the scrotum (a kind of ultrasound of the scrotum) to look for any possibility of internal swelling or fluid collection inside the scrotum, which can occur after the infection. If the ultrasound is normal, then there is nothing to worry about. Epididymo-orchitis, as the nature of the disease, also has a tendency to recur after a few months of complete recovery. If that happens, then the course of antibiotics and anti-inflammatory medication will need to be repeated. At present, a color Doppler of the scrotum is the most important investigation needed for you.

Thank you again for your time; I greatly appreciate it.

May I inquire about my epididymo-orchitis and varicocele diagnosis:

  1. Will the inflammation of the epididymis decrease, and will the epididymis return to its usual size?
  2. Do you suspect I have chronic epididymitis considering my prolonged symptoms? (I have not received a diagnosis of chronic epididymitis)
  3. Is epididymo-orchitis a persistent issue requiring long-term management, or can it be resolved with appropriate treatment?
  4. Can I consider opting for varicocele surgery to alleviate the sensation and appearance, given that the varicocele measures 3 mm?

  1. Yes, in most cases, epididymo-orchitis resolves completely and returns to normal size but the exact time for that varies from person to person.
  2. Diagnosis of chronic epididymo-orchitis can be made if the repeated scrotal ultrasound shows inflammatory changes and blood count shows signs of infection. At present, I would not say your problem is chronic without the above report.
  3. Mostly it resolves with treatment.
  4. Varicocele surgery will not be done during the inflammatory period of epididymo-orchitis. There are only a few indications for surgery for varicocele those are persistent pain or dragging sensation in the inguinal area or decrease in sperm count. It is better that you meet a urologist near you to decide if surgery is needed in your case because the need for surgery will also depend on physical examination by the urologist.
