Hello doctor,

I am a 17-year-old male from India. At this time my exams are going on so I cannot visit any doctor. Kindly help me.

I am feeling some health problem symptoms for about a week like excess sleep, short breath. When I lie down on the bed it get worse some times. I have a feeling of rubbing of knee bones while walking and also have waist pain.

Yesterday, I took Modafinil 100 mg tablet as suggested by my friend. But my symptoms gone worse. I have trouble breathing lying down, throat pain and breathing trouble, high blood pressure, blood in stool, pain in backbone, excessive waist pain, muscle and joint pain. Today I got relief from some symptoms, but still, I am feeling breathing problem, back pain, rubbing of knee bones, sweating, tiredness, sleepiness, mind goes blank and black in front of eyes when I standup suddenly. Please help.

Well, from the information it appears you have an acute stress reaction. But I need more data before reaching that conclusion as you are experiencing breathing problems on lying down and blood in your stools. Did you experience similar symptoms in the past? Do you have any past medical history? Are you feeling stressed out due to exams? Breathlessness on lying down persists for how long and how is it relieved? Do you use any recreational drugs? And most important of all do not take any drugs like Modafinil without prescription it could be dangerous. Do you experience these symptoms during each exam? because it appears your symptoms are related to stress and anxiety of exams. Please tell everything, hide nothing it will be confidential.

No, I am not stressed due to exams. It is true that I was trying to study hard before that incident of Modafinil. I am in class 11 and preparing for IIT JEE. I did not felt any such symptoms ever before. I am a experiencing breathing problem for a week before taking Modafinil. Now it is quite fine but still, sometime I feel dryness in my neck as if it is trying to close my windpipe and I start struggling with breaths. It goes fine after five to six minutes automatically or I need to pull breaths during these minutes.

I am a healthy male of 17 years. My height is 5'7" weight is 121 lbs. I am not on any type of medication. As per the stress is concerned. Once I have a stress disorder in which a severe pain was occurring inside the brain behind the right eye only along with photo and phonophobia. But I got relief by Amitone 10 mg prescribed by the doctor. I never had blood in stool and shortness or heaviness in chest ever before. Neither any neck problems, no heart or BP problems. I also get a sensation of racing of heart whenever I suffer from shortness of breath.

By reading your history it appears your symptoms are psychological or stress-induced more like a panic attack or stress reaction. Still, to be on a safer side I would like to advise you to visit a physician and get yourself completely checked with all routine investigations including ECG (electrocardiography). In the meantime you can take tablet Ciplar 10 mg single tablet when needed. Do not take more than three tablets per day and ensure minimum six-hour gap between two doses.
