What is the major cause for fatty liver?

Hi doctor,
I am a male, a vegetarian, and a non-alcoholic with lipoma. My blood sugar level is 130 mg/dL, and my blood pressure is normal. I had semen contact through the surface for three years, ten years back. I have had STD signs like bleeding in the urine, white anal discharge and itching in private parts for four to five months. I suspect fatty liver with infection. My sonography of the abdomen is normal. I took HIV ELISA fourth-generation test, HPV oral swab test, hepatitis B surface antigen test, HIV PCR test, HIV antibody test, HCV antibody test, chlamydial antibody IgM and IgG test, immunological test for syphilis, treponema pallidum antibody test, HSV type 1 and type 2 antibody IgM and IgG test, and gonorrhea test. All the test results were negative. What is the reason for my liver damage?

Did you have unprotected oral or anal sex with an infected partner? Please share your symptoms and past medical history. There may be other reasons for bleeding in the urine, such as urinary tract infection or urinary calculi. In addition, there are different causes of the fatty liver, such as consumption of fatty foods, obesity, medicine, and early nonalcoholic steatohepatitis (NASH). Therefore, I suggest you the following tests:
1. Plain CT (computed tomography) scan of abdomen and pelvis.
2. CBC (complete blood count) test.
3. ESR (erythrocyte sedimentation rate).
4. Liver function tests.
5. Renal function tests.
6. CRP (C-reactive protein) test.

When I was rubbing my penis during masturbation in the toilet seat I was exposed to the sperm produced by another boy. It happened multiple times in the six years, as I was not aware. So I got STD. But I did not involve in any sexual activity. I do not have warts. My hepatitis B surface antigen test, HCV antibody CMIA test gave negative results. My doctor told me that I have liver swelling but was not admitted in the hospital. There was no inflammation according to my cancer screening report. I have attached the reports.

From the reports attached (attachment removed to protect the patient's identity), a high level of CA (carbohydrate antigen) 19-9 indicates pancreatic cancer or cirrhosis, or gallstones. These reports are not helpful for confirmatory diagnosis of cancer. Instead, it is beneficial to determine the severity of cancer and the effectiveness of cancer treatment. Have you taken any treatment for the conditions mentioned above? Your CEA (carcinoembryonic antigen) level is normal, so your condition is not related to cancer. There is currently no government-approved test for HPV in men. Also, the possible exposure is around ten years from now. So it is unlikely to have STD (sexually transmitted disease) unless you have had intermittent exposure since then. Please send all your medical reports.
