Hello doctor,

I am experiencing pain and swelling in my penis foreskin, and the discomfort has become quite severe. The area feels swollen, tender, and painful, especially while touching, retracting the foreskin, or during urination. I am also having urinary problems, including burning sensation, difficulty passing urine, and discomfort while trying to empty the bladder. Sometimes the foreskin feels tight, and the pain makes it difficult to walk comfortably or carry out daily activities normally. Please tell me, why is this happening?

Painful swelling of the foreskin, along with difficulty passing urine, can happen due to the following reasons,

  1. Balanoposthitis.

  2. Tight foreskin (phimosis).

  3. Fungal or bacterial infection.

  4. Irritation.

  5. Uncontrolled blood sugar/diabetes.

Since you are also having severe pain and urine problems, this should not be ignored because swelling can worsen and make urination more difficult.

  1. Do not forcefully pull back the foreskin, and avoid sex or masturbation until recovery.

  2. Wash gently with lukewarm water only, keep the area dry, avoid harsh soaps or powders, and drink enough water.

Depending on the examination, treatment may include antifungal or antibiotic creams/tablets, pain-relief medicines, and sometimes medicines to improve urine flow if swelling is significant. If you develop an inability to pass urine, fever, pus discharge, black discoloration, or rapidly worsening swelling, urgent medical evaluation is needed because severe infection or tight foreskin complications may require prompt treatment.
