There is pain during defecation. There is slight bleeding on paper. No hard stool or constipation. I have this from the last three weeks.

I have checked your photo (attachment removed to protect patient identity). The differentials for this lesion include Fournier's gangrene (necrotizing tissue), Crohn's perianal disease, malignancy is a low possibility but on the list.

My advice:

  1. Seek advice in an outpatient surgery clinic as physical examination is required.
  2. Blood glucose level and HIV status should be checked to exclude immunity issues. Please provide me with the results if already done.
  3. Antibiotic therapy with Ceftriaxone 2 g per day for 1 week (after consulting your treating physician ), Brufen 600 mg as a painkiller when required and daily dressing for the lesion is required.
  4. If there is high fever or disturbed conscious level medical assessment in the ER would be necessary.
  5. Follow up after one week.
