Hello doctor,

I am 23 years old and have an issue with my penis. I am including multiple pictures. I am not sexually active, so we can rule out these things. My two main problems:

1. After masturbation, the foreskin becomes cracked and really painful for a couple of days.

2. I have white patches on the head of my penis including the frenulum.

What could it be? I do not think it is yeast or something like that. Please help.

I read your query and understand your concern.

The cracking and white patches indicate changes in BXO (Balanitis xerotica obliterans). BXO is a condition of genital skin wherein it becomes thicker, and narrower and gets white patches. It can become itchy at times or cause some mild burning. Gradually, the narrow skin opening causes fissures and when these heal, it causes more narrowing. If it progresses, it may lead to phimosis and the inability to retract the foreskin completely. It may also cause a narrowing of the urinary opening or a narrowing of part of the urine tube (stricture). Most cases do not progress further. If, in the future, the foreskin causes problems, you can get circumcision done. If the urine tube becomes narrow, it can be dealt with. Currently, you can apply simple moisturizer as needed. Please get your blood sugar levels checked to rule out diabetes. Tacrolimus cream, if available, can be applied over itchy areas (may prevent progression).

Thank you for your answer.

Would you recommend a more aggressive approach now? For example, could circumcision help? I noticed it is in the glans more. The thing is, I want to prevent progress and cancer as much as possible. From your knowledge or work experience, how many cases progress to cancer? I am 23 years old and scared. I started using Vaseline on clean skin to prevent urine from touching the skin.

Circumcision can be done when needed. The disease may not progress in many cases. Cancer due to BXO is very rare and may occur after 20 to 30 years. Very few cases have been reported till now. You can try Tacrolimus cream for a few days.
