Patient's Query

Hello doctor,

Actually, I have a fear of getting diseases and I developed this fear 10 years ago. If anyone gets a heart attack, I feel like it would happen to me. I have thoughts in my mind every time related to this fear, and I pray that nothing should happen to me. I get these thoughts repeatedly. I was taking Paxidep 25 mg as per my doctor's advice but without consulting with the doctor, I started taking 12.5 mg with this dosage I had no problem and I continued it for around four to six months. But after six months, I started having the same issue once again. Nowadays, I am suffering from severe depression. I have no control over the thoughts in my mind. I feel very sad, and emotional and I cry daily. Please suggest what I should do. Currently, I am taking Fluoxetine and half of 0.25 mg Lonazep and half of 0.25 mg Lonazep in the evening. Kindly help.

Hello,

Welcome to icliniq.com.

I am here to give you the best care possible.

You can discuss with your treating psychiatrist regarding increasing the dose of Fluoxetine and if no improvement after sufficient trial is noticed, then the dose and duration can be changed or augmented. You can also engage in psychotherapy like CBT (cognitive behavioral therapy). Some simple steps that can be of help are as follows:

  1. Meditate daily for at least half an hour in the morning as this will help you relax and focus.
  2. Follow a flexible routine, exercise regularly and take a balanced diet rich in antioxidants and omega fatty acids.
  3. Share your feelings with family or close friends as sharing helps a lot.

Remember every problem has a solution. It is just that you need to try solving it and remember that help is just a step away.

I hope this helps. Feel free to get back if you have any further queries.

Thank you.

Investigations to be done

A thyroid function test (TFT) needs to be taken.

Differential diagnosis

The differential diagnosis could be major depressive disorder (MDD), generalized anxiety disorder (GAD), or health anxiety disorder.

Same symptoms don't mean you have the same problem. Consult a doctor now!
