Hello doctor,
I am a male of 29 years old with a height of 5.5 feet and a weight of around 152 pounds. I am not under any medication. I am not married and have never had sex. Since the past few months, some greyish-black patches have been developing on the glans of my penis. It is also around the opening. Is this a cause for concern? I masturbate (using coconut oil as a lubricant) three to four times a week. During my college days, I used to masturbate daily. Since then, the sensitivity of the penis has decreased significantly. The intensity of orgasms too decreased, and for the past five to six years, I have not experienced an orgasm completely. During the climax, there is buildup from the lower bottom of the penis, but just when it is ready to reach the tip of the penis, it withers away without an orgasm. There is no problem with ejaculation. Ejaculation happens in a normal way. Around two to three years back, on checkup with a urologist, he had given an ointment and Wisenerve tablet, citing that the foreskin is slightly tight on retraction.

The only spots that I can see are toward the right lower half of your penile head. This condition is called penile melanosis or hyperpigmentation. It is a harmless condition and occurs when melanin (brown pigment) forms deposits in the surface layer of the skin. It is not anything to be concerned about and does not require any treatment. It is not going to interfere with your sexual life or cause any other issues. Unfortunately, there is no way to predict whether this will stop here or if a person will develop multiple spots in other places. With regard to your ejaculation issues, I have a few details I would like you to answer: Are you diabetic? Do you smoke? Have you had any surgeries in the past? Do you have any difficulty in passing urine, such as: straining to pass urine, urgency, the feeling of incomplete, or emptying of the bladder?

Thank you for your reply. I am not diabetic and do not smoke. I had no surgeries. There are no issues while passing urine and no issues in ejaculation except the orgasm part.

As I said, you do not need to do anything for the discoloration seen. It will be the same, or a few more spots may come up. With regards to your ejaculation concerns, it is not anything major you should worry about either. With time, and as you become sexually active, it will get better. This is what I suggest you do for now:
1. Limit masturbation to only a couple of times a week.
2. Use techniques such as squeezing or start-stop to delay ejaculation.
3. Take tablet Tadalafil 5 mg once at night for a month (then stop).
