Hello doctor,

I am 31 years old. For the last four years, I have noticed that my penis has shrunk by about 0.59 inches after I started using pills after a hair transplant. For two years, I have had erectile dysfunction. Previously, I have used antibiotics for COVID-19 and viral and bacterial infections. My test results show prolactin levels of five to ten percent higher than normal. Also, I have been masturbating since I was ten years old. What is the reason for my complaint? Kindly advise.

Hair transplant medications can shrink the penis, which cannot be reversed, and your penis will remain smaller. The elevated prolactin levels also indicate that hormonal changes in your body lead to shrinkage. Do you still take the medications? I suggest you stop taking Cabergoline and start the anti-compulsive medication to reduce your urge to masturbate and estradiol hormone pills, which will shut down your sexuality.

Treatment plan

I suggest the below treatment plan. Take Tablet Dutasteride 0.5 mg once daily, Tablet Estradiol Valerate 2 mg, once a day, and Tablet Clomipramine 50 mg three times a day. These tablets have to be taken for at least five to ten years and then taper down the doses by 10 % per month.

Regarding follow up

Have a follow-up every 15 days.
