Patient's Query

Hello doctor,

I have a medical issue that seems to be getting worse over time. Recently, I started to feel my heartbeat rapidly increase more frequently. I started to shake and get sweaty or clammy. My vision will start to close, and I feel dizzy. I can make it pass by sitting still, taking deep breaths, and drinking water, milk, or tea, and the symptoms will subside within ten minutes. When I was younger, I passed out a total of seven times with this, and I saw a specialist and was told that sometimes people just pass out, but it has become more frequent. My mother suffered a double brain aneurism, and my father has had three heart attacks, and they both have angina. I take medications Rizatriptan and Amitriptyline.

Hello,

Welcome to icliniq.com.

I thoroughly read your query and understand your concern.

Based on the symptoms you described, there are a few potential differential diagnoses to consider. These include:

  • Cardiac arrhythmias: These are abnormal heart rhythms that can cause a rapid or irregular heartbeat, leading to symptoms such as dizziness, fainting, and sweating. Given your family history of heart disease, this is a possibility that should be evaluated by a medical professional.
  • Vasovagal syncope: This is a condition in which a sudden drop in blood pressure can cause fainting, dizziness, and sweating. This can be triggered by various factors, including stress, dehydration, and prolonged standing.
  • Panic attacks: These are sudden, intense periods of fear or anxiety that can cause physical symptoms such as rapid heartbeat, sweating, and dizziness.
  • Hypoglycemia: This is a condition in which blood sugar levels drop below normal, leading to symptoms such as sweating, dizziness, and fainting.
  • Other neurological or endocrine conditions: There are a variety of other conditions that can cause symptoms similar to what you are experiencing, such as seizures or thyroid disorders.
  • I also suggest you the following investigations:

Ask your general physician to check for postural hypotension and get an ECG (electrocardiogram) and ECHO (echocardiogram) done. You can show that ECG to me here. Meanwhile, drink more water as at your age, I would consider my first differential diagnosis to be postural hypotension.

Feel free to contact me for further queries.

    Regards.
