Hello doctor,

My triglycerides have been increasing since last year and I having issues with breathing problems on exertions like sudden small actions and sweating, and my heart rate increased up to 150 beats per minute. I did the echocardiogram and the treadmill stress test which came back normal. I want to consult on increased triglycerides 5.47 mmol/L and hemoglobin is low 12 g/dl.

Did you give a fasting sample for lipid profile testing? If your sample was not fasting then the lipid profile should be tested again in a fasting state. If it was a fasting sample, then triglycerides are moderately elevated. To address this issue of elevated triglycerides, the initial approach is dietary and lifestyle modifications. You should reduce the consumption of simple carbohydrates, sugars, oily and fatty food as low as possible. Also, you need to start physical activities like walking or brisk walking at least 30 minutes a day. If there is any alcohol consumption, it needs to be reduced as it can elevate triglycerides. You should repeat the lipid profile after four to six weeks after eight hours of fasting at least and if there is a response for the same, then a healthy lifestyle needs to be continued, or else medicine will have to be added. Also, your symptoms may be related to intolerance to physical activities due to a sedentary lifestyle as your echo and TMT didn't show any organic heart disease. Regular physical activities may reduce these symptoms. Also, you can get your lungs evaluated by pulmonary medicine once especially if symptoms are severe or you are not able to perform regular physical activities. Hemoglobin is in the lower normal range. To find out the cause, you need to get a serum iron profile, peripheral smear, vitamin B12, and folate testing done. You should have an iron-rich diet like green leafy vegetables, dates, etc. I hope this helps you get back if you have any more doubts or more information to share.
